Does a council's RFO have to be appointed every year at the annual meeting?

No.  The RFO role is usually part of the full job title of the Clerk and, like any other job, the individual retains the role and title until they resign/retire etc.

The list of business to be included in the agenda for the annual meeting appears in the model standing orders.
