How can a member of the public get an item on the agenda of the Annual Parish Meeting – especially on a topic the chair of council would prefer not to discuss?

The Annual Parish meeting is a meeting of electors and not a meeting of the council so you should be able to raise anything you like (well, within reason!).  We invite items for the agenda from members of the public for several months before the meeting an also offer an open session at the meeting itself.  If your local council is running the meeting (many do) simply ask the Clerk to add it to the agenda.,
